Online Registration Documents
All registrations must have proof of residency, birth certificate and immunizations or the registration application will be denied.
The following registration documents may be required, if applicable, to complete the registration process.

Immunization Exemption
The immunization exemption form is only for families that have medical, religious, or ethical/moral reasons for not vaccinating their child. Please complete only the portion that pertains to your child.
Residency Affidavit
The residency affidavit only applies to students not living with a biological parent or legal guardian or when a student and his/her parent(s)/guaradian(s) are residing with another family
The residency affidavit will need to be notarized.
Technology Agreement
This form is not required for student registration, but is required for a student to be issued a district device.
Athletic Intent to Participate Form
Any student entering Southmoreland School District who desires to participate in Interscholastic Athletics should complete this form.

Please review the following instructions for successfully completing a fillable PDF form:
- Use only the latest version of Adobe Reader to complete fillable PDF forms. Macintosh and Windows versions of the free Adobe Reader are available from Adobe at http://get.adobe.com/reader/.
- Before completing the document save the form (PDF format)to a location on your computer.(Example: Desktop or Documents). Once you have saved the form to your computer, you are ready to complete the form.
- Open the fillable form and complete form.
- After you have completed the form, save a final version of the file to your computer. When ready, attach and email the form to registrar@southmoreland.net
