ACCEPTABLE TYPES OF PROOF OF RESIDENCY:
- Utility Bill (electric, gas, water, sewage, trash, cable, internet (document must contain your name, address and a date within the last 30 days)
- Current Lease, Rental or Sales Agreement
- Notarized Residency Affidavit (if living with someone other than a parent/guardian)
